Needed to Attend SonShine Preschool:
- Online Enrollment Form
- $125 enrollment fee will be billed after we receive your form
- You will receive a confirmation text within a week after paying enrollment fee.
- Immunization Record - Due before your child's first day. We are happy to look up your child's immunization record in the secure Colorado database. (If opting out of immunizations, please complete and submit the Immunization Exemption Form)
- General Health Appraisal Form Due within 30 days of your child's first day

Medication Forms
Please note that we can not possess or administer any medications without completed documents. Medications must have the original personalized pharmacy label attached.
- ASTHMA (if needing meds at school): Respiratory Health Care Plan
- ALLERGIES (if needing meds at school): Allergy Health Care Plan
- SEIZURES (if needing meds at school): Seizure Action Plan
- DIABETES: Diabetes Action Plan & Insulin Pump Addendum
- Other special medical needs: Individualized Health Care Plan
- For any other medication needed at school (other than for asthma /allergies/ siezures / diabetes): Medication Administration Authorization Form
